Compassionate Care for Your Home
In our Home Health Care division, where your well-being is our utmost priority, we understand that seeking home health care services can be a significant decision, often driven by the need for specialized and compassionate care within the comfort of your own home.
We service clients currently using Molina, Passport, Aetna, ESP, and those who do not currently use a healthcare agency can be provided with a Personalized Care Plan.
How to Request a Home Health Aide
(For you or your family member)
*Your family member may be able to become your aide through us by applying for a home health aide position.
If currently with a health careagency:
Contact the Council on Aging (COA) to see if you are eligible for a home health aid and, if so, how many hours you are allotted.
Once COA has confirmed your eligibility, have the agent contact Eastern Home Health and request home health aid services.
If not with a health care agency:
Call (513) 421 - 4666 and ask to speak with the home health division about a personalized care plan.
Once you are transferred to the home health line, express that you are inquiring about a Home Health personalized care plan.
